Duties and Requirements:
Implement Plan of Care (POC) with client or client and parent/guardian.
Attend staff meetings/supervision and training per supervisor's requirements.
Maintain contact with referring agency every week to discuss updates, progress, concerns, or provide feedback.
Adhere to all agency policies and procedures and complete other tasks deemed necessary for the successful operation of services.
Complete all required documentation which includes, but is not limited to, tracking of the POP and related paperwork.
Must complete a TB screening within 30 days.
Must complete and maintain HIPAA Certification within 90 days of starting.
Maintain CPR and First Aid Certification at all times.
Must be found eligible via a background check with the respective state, FBI, and Child Protective Services or Social Services.
Must comply with Medicaid and Licensure required training.
